Athletics

Parent Permission Form

All participants in the interscholastic athletic program will be required to have on file with the coach a signed Parent Permission form prior to participation. A separate form is required for each sport.

It is the responsibility of the head coach to provide these forms and to be sure that they are completed and returned.

1. Prior to the imposition of any suspension/dismissal from participation in any athletic activity, the student athlete and his/her parents/guardians will be given the opportunity to discuss the charges with the appropriate administrator and/or Athletic Director.
