The Board recognizes that, by virtue of the critically important work performed in the School District on behalf of children, there are many athletic coaches who, on a daily basis, are deserving of significant recognition.  Indeed, the District would need to build a city in order to have enough facilities to name for each and every one of those individuals.  In that context, this policy acknowledges those many heroes who have, and will continue to have, a significant and positive impact on the children of the Fallsburg community.  For those who have made extraordinary contributions or experienced superb accomplishments as members of the Fallsburg school community, the District has established a Wall of Coaching Distinction Plaque as a place of honor.

In those instances where an individual’s contribution or accomplishments stands out as unique, enduring, and impactful, the Board of Education will consider adding the names of deserving individuals to the Wall of Coaching Distinction Plaque. The procedure for consideration of such naming honors is as follows:

1) No later than September 30 in any school year, a completed application (see attached application) will be submitted to the Board of Education for consideration.
2) The completed application will be forwarded to the Board of Education Policy Committee for initial determination and recommendation to the Board of Education a final decision to add an individual’s name to the Wall of Coaching Distinction Plaque.

CRITERIA
Criteria to be utilized at each phase of the consideration process shall include, but not be limited to, the following:
(1) Retired from coaching FCSD sports
(2) Long-standing, exemplary coaching service to the District, 5 year minimum;
(3) Outstanding contribution to a school or the District as a whole;
(4) Significant accomplishment in coaching at the local, state or national level

The Fallsburg Central School District Wall of Coaching Distinction Plaque was created as a means of recognizing great coaching accomplishments. Many Fallsburg coaches have made great contributions to the lives of student-athletes over the years in various sports and sporting programs. The Fallsburg Central School District Wall of Coaching Distinction plaque honors the contributions and accomplishments of those individuals, who, through coaching, have made a difference in the lives of students,

GENERAL RECOGNITION
Nominees must exemplify the high standards of effective coaching, leadership, ethical conduct and moral character. To be considered for this prestigious honor, a nominee must have been a coach in the Fallsburg Central School District for a minimum of 5 years. All candidates will be judged on their significant and/or long term coaching contributions to the Fallsburg Central School District. While many have served as coaches at Fallsburg, their accomplishments must be worthy of consideration. 

INSTRUCTIONS
All information on this form must be completed and supporting letters of recommendation must be received at the Office of the Superintendent of Schools by September 30 of each year. The completed nomination form and support materials should be sent to:

Once the completed nomination form is received by the Fallsburg Central School District, all actions necessary to the selection process shall remain confidential.  All information submitted shall be retained by the school district.  If you have questions, please call the Superintendent at 845-434-6800, ext. 1215.

SPECIFIC ITEMS NEEDED TO COMPLETE THE APPLICATION
Letters of Recommendation:  A completed nomination form should be accompanied by three letters of recommendation: one from the nominator and two from appropriate individuals (former athletes) who can speak knowledgeably about the nominee’s accomplishments.
Support Material:  A limited number (5-7) of newspaper clippings, magazine articles, and other written media should be included to help substantiate the coaching accomplishments and contributions of the nominee. These materials should focus on the individual’s accomplishments and contributions in chronological order.

In cases where other support material is difficult to obtain, additional letters may be used to substantiate the individual’s accomplishments or contributions. Letters of recommendation for other awards or honors should not be used to substitute for this requirement. In completing nomination materials, please observe the following: 1) All copies should be 8 ½” by 11”; 2) nominators should sign their names in the space provided; 3) do not use highlighters to point out specific facts-either underline or draw arrows to mark copy; 4) please type all material so that copy is legible; 5) do not use notebooks with plastic sheet protectors; 6) put names on all photos.
