Hall of Fame Nomination Form: Click Here

Hall of Fame Website

The Fallsburg Central School District Hall of Fame was organized as a means of recognizing, preserving and promoting the heritage of the Fallsburg school community.  Many individuals have made extraordinary contributions and have experienced superb accomplishments as members of the Fallsburg school community. The Fallsburg Central School District Hall of Fame honors the contributions and accomplishments of individuals who are worthy of district-wide recognition

Nominees must exemplify the high standards of citizenship, leadership, ethical conduct and moral character.

The categories of student, staff, administrator and community member are included on the nomination form for expediency in understanding the primary area in which an individual is to be considered for his or her achievements. The group identification is for screening purposes only and to assist those making the nominations to understand the type of information required.  Once nominees are inducted, they are not categorized, but are all recognized members of the Fallsburg Central School District Hall of Fame.   All candidates will be judged on their significant and/or long term contributions in their lifelong pursuits.  These pursuits include but are not limited to: fields of study, careers, athletics, service to community (at local, state and national levels), professions, academic accomplishments, and others.

While many have served their communities over a long and distinguished career, their accomplishments must be worthy of prominent recognition for Hall of Fame consideration.  Nominees for the Fallsburg Central School District Hall of Fame will either be approved and recognized as a Hall of Fame Inductee or will be placed in the Holding for Future Consideration Category by the Fallsburg Central School District Selection Committee.  This Selection Committee is made up of: 2 students, 2 staff members, 2 administrators, 2 community members and 2 members of the Board of Education.  The Superintendent serves as the Selection Committee Chair.

The Holding for Future Consideration Category means a nominee may be considered again for induction for three consecutive years and that additional information and supportive material would be helpful before future consideration.

All candidates for the Fallsburg Central School District Hall of Fame must be 1) a current or former student, 2) a current or former staff member, 3) a current or former administrator or 4) a current or former community member, in order to be considered for induction.

All information on this form must be completed and supporting letters of recommendation must be received at the Office of the Superintendent of Schools by February 16, 2024 for the candidate to be considered for the 2023-2024 school year.  The completed nomination form and support materials should be sent to:

Fallsburg Central School District
Superintendent’s Office
Hall of Fame
PO Box 124, 115 Brickman Road

Once the completed nomination form is received by the Fallsburg Central School District, all actions necessary to the selection process shall remain confidential.  All information submitted shall be retained by the school district.  If you have questions, please call the Superintendent at 845-434-6800, ext. 1215.

Photograph:  A black and white photograph of the nominee must accompany the completed nomination form.  The name of the nominee should be printed on the back of the photograph.

Letters of Recommendation:  A completed nomination form should be accompanied by three letters of recommendation: one from the nominator and two from appropriate individuals who can speak knowledgeably about the nominee’s accomplishments.

In cases where other support material is difficult to obtain, additional letters may be used to substantiate the individual’s accomplishments or contributions.  Letters of recommendation for other awards or honors should not be used to substitute for this requirement.

Support Material:  A limited number (5-7) of newspaper clippings, magazine articles, and other written media should be included to help substantiate the accomplishments and contributions of the nominee.  These materials should focus on the individual’s accomplishments and contributions in chronological order.
Candidates whose careers ended 50 or more years ago and who are deceased will be considered against the accomplishments of their contemporaries, as much as possible.  It is suggested that additional support material be included, such as letters from individuals who had some involvement with the nominee.
In completing nomination materials, please observe the following: 1) All copies should be 8 ½” by 11”; 2) nominators should sign their names in the space provided; 3) do not use highlighters to point out specific facts-either underline or draw arrows to mark copy; 4) please type all material so that copy is legible; 5) do not use notebooks with plastic sheet protectors; 6) put names on all photos.

The Hall of Fame Induction Ceremony will take place at the Fallsburg Alumni Luncheon in July 2024. Inductees will be notified of their selection by the end of March, 2024.
